A Whole New World?

Just curious if most writers of paranormals are working within the real world, and asking readers to suspend normalcy by buying into one or two paranormal elements ... or are you creating whole new worlds of paranormal activity? :) I find myself a member of the first group, but it's sometimes a fine line to walk. I have HUGE respect for people in the second group, who can create whole new worlds, a la J. R. R. Tolkien. And for those folks in both categories ... what were your challenges as you worked to create your story?
